                 A medical scribe is a doctor's personal assistant who has been trained to record doctor-patient interactions during medical examinations. A medical scribe records patient visits into an electronic health record (EHR) in real time, as directed by a doctor, and uses the EHR to track the patient's progress. A medical scribe typically works as a contracted service or is employed by a healthcare organization, a doctor, or both.

Real-time documentation of a patient's visit is done by a professional known as a medical scribe who collaborates with a healthcare provider. The scribe documents the patient's medical history, symptoms, results of any diagnostic tests, and the results of any physical examinations. The scribe also records orders for prescriptions, lab work, and follow-up appointments.
